It is important for Caring For You Home Care to provide individuals with CONTROL, CHOICE, and FREEDOM. Individuals should have control over all areas of their life, the choice to decide everything about their life, and the freedom to have the same rights as all other members of the community. While it is important for the individuals to have these things it is important to have the family and supporter aware of these plans so that they can be supported properly.
Identifying and addressing the Social Determinants of Health(SDOH), if needed, is also a main focus when working with individuals. SDOH are conditions in the places where people live, learn, work, and play that affect a wide range of health and quality-of life-risks and outcomes. The goal is to help improve the quality of life when addressing these issues. We believe in doing more than meeting basic physical needs. Each person we serve is special and a priority! They are someone’s mother or father, grandparent or sibling. With that in mind, we care for them as if they were our own family and treat them with respect and dignity at all times.
